Application Check List

Items Needed for On-line Application

Once you have completed the loan application, please submit and I will be in contact with you very shortly. Below please find a list of paperwork necessary to proceed with your mortgage.
  1. Last (2) years W2 and/or 1099 forms
  2. Most recent pay-stub showing year to date income totaling one month (Unless self employed, then please use item #7)
  3. Most recent Checking, Savings, Retirement, Brokerage, 401k, ect. account statements (ALL pages)
  4. (If a Refinance) Copy of the survey from original closing package
  5. (If a Refinance) Name, account number, and contact number for your current mortgage
  6. Insurance Declarations Page, OR Insurance company name, agent and contact number
  7. Federal Tax returns for the previous 2 years (If self employed or receive commission income)
  8. If divorced, we need a copy of the divorce decree
  9. Copy of your Driver's Licence
  10. Copy of Lease Agreements on any rental properties & Complete Copy of Bankruptcy Papers (If Applicable)
